Central Virginia Advanced Directives Collaborative – who we are, what we do, and where we are going.

WHO?

The Central Virginia Advance Directives Collaborative is serving as a convener of interested parties to broaden and strengthen a community-wide conversation of Advance Directives 

To date the collaborative consist of 50+ members representing 20+ organizations, government and business entities and both area hospitals.

WHY?

In the region, there are and have been many stand-alone groups that have had successful programs on increasing awareness of the importance of advance directives but none have resulted in a significant uptick in actual submissions of completed directives to healthcare providers.  It is hoped that the larger scale of a community wide effort along with a focused goal on increasing actual submissions of completed directives, we will be able to get the desired results.

Our vision is to have the majority of the 18 years of age and older population of this region file Advance Medical Directives with their health care providers. We are inspired by La Crosse, Wisconsin, where 96% of people who die there have had Advance Medical Directives on file.

PURPOSE

The purpose of the Advance Directive Community Collaborative is to increase public awareness and submissions of medical directives through: 

            Education

            Advocacy

            Data

            Physician, pastoral and community partnerships

VISION

Have majority of population of this region file Advance Medical Directives with their health care providers. (Inspired by La Crosse, Wisconsin, where 96% of people who die have signed Advance Medical Directive)
